Abstract

|
목적: 본 연구는 유치도뇨간호를 위한 이전(초판)의 근거기반 임상간호실무지침을 업데이트하기 위해 수행되었다.
방법: 국제 표준을 기반으로 22 단계로 지침이 수정되었다. 개정에 활용될 실무지침의 질은 Appraisal of Guidelines for Research and Evaluation II 로 평가하였다. 개정될 실무지침 권고안 초안의 내용 적절성과 임상실무에서 적용 가능성의 평가는 RAND Corporation의 RAND/UCLA Appropriateness Method 의사결정 방법을 사용하였다. 갱신용 지침으로는 4개의 지침이 활용되었다.
결과: 유치도뇨간호 실무지침 개정은 9개의 영역과 134개의 권고사항으로 구성되었다. 각 영역의 권고사항 수는 다음과 같다: 사정 4개, 물품 20개, 유치도뇨 삽입 11개, 유치도뇨관 유지 52개, 유치도뇨관 및 소변백 교환 4개, 유치도뇨관 제거 9개, 합병증 관리 22개, 교육과 상담 5개, 그리고 기관의 지원 7개. 권고안의 권고등급 A는 8.2%, B는 38.1%, C는 53.7%이었다. 이 중에서 주요 개정은 11개(8.2%)의 권고안이었다. 29개(21.6%)의 권고안이 새로 추가되었다. 30개(22.4%)의 권고안은 단어나 문장의 변경 또는 추가와 같은 작은 개정이었으며, 13개의 권고안이 삭제되었다.
결론: 개정된 실무지침은 한국에서 유치도뇨간호를 위한 근거기반 임상간호실무지침으로서 역할을 할 것으로 기대된다. 이 지침은 건강관리제공자, 환자 및 돌봄제공자에게 유치도뇨간호를 관리하는데 도움되는 정보를 제공하여 환자관리를 개선할 것이다.

Purpose: This study was conducted to update the existing evidence-based nursing clinical practice guideline forindwelling urinary catheterization (IUC).
Methods: The guideline have been revised in 22 steps based oninternational standards. The quality of the practice guidelines to be used for revision was evaluated using theAppraisal of Guidelines for Research and Evaluation II. The evaluation of the content appropriateness andapplicability of the draft recommendations of the revised practice guidelines was performed using theRAND/UCLA Appropriateness Method, a decision-making method developed by the RAND Corporation. Four guidelines were used for the revision.
Results: The updated nursing practice guideline for IUC consistedof 9 domains and 134 recommendations. The numbers of recommendations in each domain were: 4Assessment, 20 Equipment, 11 Catheter insertion, 52 Catheter maintenance, 4 Catheter and drainage bagchange, 9 Catheter removal, 22 Complications management, 5 Education and consult, and 7 Hospital support. The recommended grade was 8.2% for A, 38.1% for B, and 53.7% for C. Among these, the major revision wasdone in 11 recommendations (8.2%). A total of 29 recommendations (21.6%) were newly added. 30 (22.4%)recommendations had minor revisions such as changes or addition for some words or sentences, and 13 (9.7%)recommendations were deleted.
Conclusion: Revised nursing practice guideline is expected to serve as anevidence-based practice guideline for IUC in Korea. This guideline will provide health care providers, patients,and caregivers with information to help manage IUC, leading to improved patient outcomes.
